A big problem is that your questions have more than one answer, especially one and two. Us cosleeping moms often "use" a matress to rest the baby on while nursing, and there are teas that help with engorgement.
Also, women experiencing cracked nipples have sooooo many more options than just a nipple shield. Use your own breastmilk, for starters.
